The aim of our group is to develop a dialogue between group members and the practice in order to work together to improve our services and provide the best possible care to our patients.

Patient Participation Group
The Patient Participation Group, or 'PPG', is a group of patients who volunteer to be the ‘listening voice of the patient’. We work alongside the practice to support, offer advice and ensure the patient’s voice is heard.
The group allows patients to provide feedback directly to the practice, leading to constructive discussions and improvements to services. The group’s opinions are listened to, as the people who use the services are the best people to tell us what works and what improvements we could make.
Any patient or registered carer of a patient may apply to join the PPG. The current terms of reference state that there is a maximum of 20 members. Members are asked to sign a Confidentiality Statement. If you would like to join the PPG, please fill out the registration form on this page and submit it to us.
The meetings are held at the Practice, usually at 5pm. They last for about 60 minutes. At present, these meetings are held on a Wednesday and are usually bi-monthly.
Members are expected to attend as many meetings as possible or send their apologies in advance of the meeting.
Agendas and minutes are usually circulated by email.
Within the meetings we would not discuss personal matters and primarily focus on discussions around patient experience and updates on patient focused changes that we are planning.
If your Doctor is involved in the subject for discussion, then they may attend the meeting. The content of the meetings does not form part of your medical record.
We will only ask questions relating to the practice and the services we provide. There is no expectation to disclose personal information.
As always, all information you provide to us will be kept safe and secure. They will only be used for the purpose you have provided them for and they will not be shared with anyone else.
If at any time you change your mind and no longer wish to be involved, let us know in writing and we will remove your details from the group members list.
The PPG has a Facebook page called 'Sleaford Medical Group Patient Participation Group'. Via this page, you will find posts that you may comment on and surveys that you could take part in. Feedback in this way helps us keep in touch with more of our patients.
We contact the members mainly for the purposes of organising meetings or to feedback on meeting outcomes, via email.
We can communicate with you in different ways to suit you such as email or telephone.
